available to you through our Website is only the information
that you have requested. We also collect non-personal information
from you about your use of our Website, emails and special
promotions that we send to you. Other than the information
you provide at sign up, which you may update it from time
to time, PHN receives no information about you from Southern Ohio Medical Center.

with Southern Ohio Medical Center, both Southern Ohio Medical Center and PHN have access to and

that is either branded or co-branded with Southern Ohio Medical Center. Additionally,
Southern Ohio Medical Center is provided information about aggregate usage
of the Site. Users cannot opt-out of having their information
provided to Southern Ohio Medical Center. Southern Ohio Medical Center has agreed to honor the
applicable terms of this privacy statement with regard to
all information collected through our Website; however,
this privacy statement does not cover information that you
have directly provided Southern Ohio Medical Center or that Southern Ohio Medical Center has been
provided by any third party.

We store information that we collect through log files
and/or third parties to create a profile of our users. A
profile is stored information that we keep on individual
users that details their viewing preferences. Consequently,

user's visit to our Website, and to direct pertinent marketing

Confirmation. We send
all new users a welcoming email to verify passwords and

practices. Every quarter, as well as any time new policies
